I usually put keyword, description then title but i hear you should put title first for best search results?
The order of that meta tags doesn’t have any rules nider Google asks for some order. The only important thing is that those 3 tags shall be in head section.

Yes, you should place the <title> tag before the <description> & <keyword> tags.
Your structure should resemble this below;

The title & description tags are the most important – the keyword tags are mostly ignored by all the major search engines.

That question doesn’t entirely make sense. When you implement your meta tags into the head of your website, there is a specific place for each meta tage. The title, regardless will be shown at the top of your browser if done correctly. There is argument as to whether or not meta keywords are relevant but I always use them anyway, as it sets as a guideline to what each page is focusing on. The description is MOST important, this describes what this page is about the search engines often use it as their description too for the search results.

We personally always put the title after the Meta Description and the Meta Keywords.

Yes, Title before keyword. And remember, do a check on your keyword density for your site as well.
